Wegovy Takes a New Form: The Pill vs. The Injection Showdown!

    Wegovy

    Key Takeaways

    • A daily oral version of Wegovy, the renowned injectable weight loss medication, is now on the market.
    • The pill may have a higher propensity for side effects and slightly lower weight loss outcomes when compared to the injection version.
    • Research indicates that adhering to the oral formulation proves to be particularly challenging for many users in real-world settings.

Wegovy has transitioned from its injectable format to a convenient once-a-day oral pill.

Like its injectable counterpart, this pill contains semaglutide as its active ingredient. Patients typically begin with a low dose, gradually increasing it every four weeks, until they reach the maximum dosage for weight management. This gradual increase can help the body adjust and may alleviate common adverse reactions like nausea and diarrhea, noted Roman Fazylov, PharmD, a professor at Touro College of Pharmacy in New York City.

However, some individuals may experience less weight loss with the pill versus the injection. The side effects could also feel more pronounced, largely influenced by the oral absorption process.

How Does the Wegovy Pill Work?

The Wegovy pill is absorbed in the stomach, meaning only a small fraction of the active compound enters the bloodstream. To counteract this, the maximum dosage for the pill is set at 25 mg, significantly higher than the injectable form’s limit of 2.4 mg. This disparity may lead to heightened side effects for certain users, Fazylov explained.

A significant drawback of this oral option is its convenience factor. “For many users, it isn’t genuinely more convenient,” shared Y. Tony Yang, ScD, LLM, MPH, associate dean for Health Policy and Population Science at George Washington University School of Nursing.

“You have to take it first thing in the morning on an empty stomach with just 4 ounces of water, then wait 30 minutes before eating, drinking, or taking other medications,” Yang elaborated. “Miss that time frame? You might impact your dose negatively. For shift workers, frequent travelers, or those without consistent morning routines, the injection offers greater flexibility.”

Though the Wegovy pill and injection haven’t been directly compared, both have undergone distinct testing models, according to Andrea Traina, PharmD, senior medical director for obesity and liver health at Wegovy’s manufacturer, Novo Nordisk.

“In clinical trials, injections resulted in an average weight loss of 15%-17% over 68 weeks, while the oral version achieved around 14%-17% weight loss at 64 weeks,” Traina communicated to Verywell.

Is the Wegovy Pill More Convenient?

According to data from Novo Nordisk, patients managing side effects effectively while remaining on the drug saw an average weight loss of 16.6% after 64 weeks, with about one-third achieving at least a 20% reduction in body weight.

Nonetheless, when factoring in those who continued versus those who ceased treatment, the average weight loss drops to 13.6%, Yang pointed out. “This is consistent with our observations regarding injectable versions,” he stated. “The medication is very effective if users stay committed.”

Real-world data indicates that only 56% of conscious individuals using injectable semaglutide completed a full year of treatment, with an average weight loss of 10.9% among continuing patients.

Adherence to the oral formulation may prove even more demanding. In a separate study, just 27.2% of users maintained their commitment to this oral version of GLP-1 throughout the first year, a significant decline compared to weekly injections.

“The strict dosing guidelines present numerous opportunities for failure,” Yang noted. “Indeed, while the pill can help reach recommended weight loss objectives provided that patients remain on it, the ‘if’ is certainly significant,” he added.

Who May Benefit Most From the Wegovy Pill?

Traina mentioned that the pill might suit individuals who have a strong aversion to needles or those who travel regularly. Unlike the injection, which necessitates refrigeration, the pill does not have that requirement. Insights from Novo Nordisk’s market analysis indicate that younger individuals and those needing to shed less weight show pronounced interest in the oral version.

She further suggested that dietary adjustments could minimize the side effects of the pill. These modifications include eating slower, opting for smaller portions, stopping when satisfied, and avoiding both alcohol and high-fat foods.

Fazylov added that medications can effectively manage specific side effects. “Heartburn is a considerable concern, and we can prescribe treatments to assist with that.”

Why You Should Not Cut or Split Wegovy Tablets

Numerous users of injectable Wegovy have discovered methods to adjust doses to mitigate side effects or cut costs.

Yang cautioned against attempting this with the oral formulation. “The manufacturer and FDA emphasize that the tablet must be swallowed whole,” he clarified. “Do not cut, crush, or separate these tablets.”

The tablet’s integrity is crucial for the intended absorption function. Altering it can destroy the absorption mechanism and may lead to minimal bioavailability.

Taking the pill every other day to reduce costs or lessen side effects won’t yield positive results. “Irregular intake will result in fluctuating drug levels, compromising the required chronic GLP-1 receptor activation necessary for weight loss,” Yang added.

If patients struggle to manage the prescribed dose or maintain the regimen, Yang advised consulting their healthcare providers to consider dose adjustments or switching to an injectable option.
